Types of clinical note

A clinical note's type determines how you can update the clinical note. For example, if the type is 'Text' you can enter free text in the note.

Listed in the table below are the different types of clinical notes.

Text

You can enter free text. This type of note might be used for a management plan clinical note where you have a large amount of text to record.

Date

You can add a date from a calendar. This type of note might be used for an EDD (Expected Date of Discharge) clinical note.

Select

You can select a single option from a list. This type of note is used for clinical notes that only want a single value.

For example, the clinical note 'Medically Fit' has the options 'Yes' or 'No'.

On a mobile, 'Select' and 'Multiselect' clinical notes look the same. On a desktop, 'Multiselect' notes have circles next to each option, but 'Select' notes don't.

Multiselect

You can select multiple options from a list. This type of note is used for notes that need more than one value.

For example, the clinical note 'Safety Alerts' has the options 'Risk to Self' and 'Risk to Others'. This note allows you to select both options.

On a mobile, 'Select' and 'Multiselect' clinical notes look the same. On a desktop, 'Multiselect' notes have circles next to each option, but 'Select' notes don't.

Camera

You can take images with Nervecentre for mobile. These images are stored in the patient record, and are available from both desktop and mobile. This type of note is used to support wound management.

Select or free text

You can select a single option from a list, and/or enter text. This type is used for notes where you want to add context to the option that you chose.

On a mobile, 'Select or free text' and 'Multiselect or free text' clinical notes look the same. On a desktop, 'Multiselect or free text' notes have circles next to each option, but 'Select or free text' notes don't.

Multiselect or free text

Date and Time

You can select a date from a calendar and enter a time.

Coded Select

You can choose a single option from a list. The options in the list come from a reference table. For example, the options might come from a dataset supporting ECDS (Emergency Care Dataset).

On a mobile, 'Coded Select' and 'Coded Multiselect' clinical notes look the same. On a desktop, 'Coded Multiselect' notes have circles next to each option, but 'Coded Select' notes don't.

Coded Multiselect

You can choose multiple options from a list. The options in the list come from a reference table. For example, the options might come from a dataset supporting ECDS (Emergency Care Dataset).

On a mobile, 'Coded Select' and 'Coded Multiselect' clinical notes look the same. On a desktop, 'Coded Multiselect' notes have circles next to each option, but 'Coded Select' notes don't.

User

You can select a user's credentials to add a user's name to the clinical note. This type of note is used for clinical notes that identify who has seen or reviewed a patient.

On a mobile, you can only select yourself. On a desktop, you can select a different user.

Snomed CT

You can add terms from the SNOMED database. This type of note is used for clinical notes that need to contain SNOMED terms, and there are many SNOMED values. For example, the clinical notes 'Diagnosis' or 'Allergies'.

Bodymap

You can open and annotate a body map. You can use a 'Bodymap' clinical note to support a patient assessment, for example, when assessing a patient's lungs.

On a mobile, you only see the number of body maps images. On a desktop, you can view, add, and edit the images.

Documents

You can scan and upload a document. You might use a document clinical note when the patient attends hospital with paper information and you want this information available to all users, for example, a hospital passport.

On a mobile, you can scan documents. On a desktop, you can upload documents.

Name and address

You can add an existing name and address from the address book, add a new name and address, and edit existing names and addresses.

Citation

You can add existing information onto a clipboard, and paste it into a citation note field. You can add specific Investigation results to a citation note. Citation notes are used in forms.

Audio

You can record your voice and listen back to the audio clip.

Text with Audio

You can record an audio clip and enter text. You can listen to the audio clip and view the text.

Coded Advanced

You can select options for multiple fields in a table. You can add new items to the table. You can delete items from the table.
